Affidavit of Loss/Release of Interest

Form Information:

This is a form issued by the Department of Licensing. Its purpose is to document a lost Washington State title. It can also release the interest of the seller for the referenced vehicle. It is WSIADA's recommended best practice that this form be executed for every trade in and every purchase from a private party. It is sold as a loose, individual sheet and does not come in pad form.
